at any one hospital in any city over 50,000, you can bet the total of the annual incomes of the physicians practicing there is well beyond 100 million and I'm talking about considering the net income in their pockets.
Making that sort of money demands that they assume way more responsibility than to allow that to happen even once.

meanwhile the FDA is removing generic drugs from the pharmacies so we have to pay 100 times as much for new drugs (like Vioxx)

If i am reading that chart right i have a tough time believing the max side. I think some of these guys down here go way beyond those charts.

Those are are for base pay....like the "draw" or "guarantee" of an insurance salesman's job before commission or a coctail waitress's without tips. And those figures are for the ones that hold positions where someone gives them a paycheck, like maybe a steel worker or a clerk in a department store. But the fees they collect that aren't included in that base salary aren't there.
